Information & Referral provides information about CPWD programs and services, community resources, and referral to other services providers in our community. There is no cost to receive Information & Referral. The service is available to anyone with any disability, as well as family members and friends.
The information provided is intended to assist individuals with disabilities in connecting to the resources and services they need to overcome barriers to independent living. We can direct you to Independent Living Advisors at CPWD that can further assist you, or direct you to resources and supports in your community.

CPWD is not a crisis-intervention service provider. If you are in crisis or need immediate assistance, please see our FAQs below for links to resources that provide immediate assistance and crisis intervention.

CPWD does not offer legal services, but we can refer you to excellent resources in the community. In addition, an Independent Living Advisor can assist you in navigating access to legal services, and may be able to assist you with related issues such as skills training and accomodations.

Bridge to Justice – provides no-cost legal support to connect you to lawyers who are available to provide pro bono (free) legal advice and services.
Rocky Mountain Legal Center – provides legal services to people with disabilities on an income-based sliding scale
Disability Law Colorado – focuses on legal issues related to discrimination and civil rights
Colorado Fund for People With Disabilities – supports people needing assistance with representative payee and ABLE accounts, conservatorships, and trusts
Boulder Eviction Prevention and Rental Assistance – get advice and support on eviction and rent payments

CPWD does not offer Long-Term Care or Home Care services. However, you may need assistance applying for Medicaid first, and we can help with that. If you need to apply for Medicaid, you can apply on the PEAK Colorado website.
If you (or someone you are assisting) are already on Medicaid, you can use the following resources to find care:

PASCO (Personal Assistance Services of Colorado) is a Home Care service provider that serves a large area and has Independent Living experinece.

CPWD does not offer direct medical services, and we have no doctors on staff.
If you are on Medicaid, and looking for a disability evaluation, please visit Health First Colorado to find a doctor and request and evaluation.
If you are on Medicaid, and looking for a doctor or therapist, you can visit Colorado Community Health Alliance, (CCHA), the service/resource navigators for Medicaid participants, or call them, Monday – Friday 8AM -5PM at (303) 256-1717.
If you are not on Medicaid, need assistance applying for Medicaid first, you can apply on the PEAK Colorado website.

Getting There Guide will also help you find transportation providers who can take you to your destination, showing the ones that cross county lines or meet other transportation requirements.

If you are wanting to find work, but are concerned about losing benefits, we can help. We staff Community Partner Work Incentive Counselors (CPWICs), who are experts in helping you navigate looking for work while also protecting your benefits. CPWD has a Career Coach who is available to help you with employment-related goals including career exploration, resume preparation and review, interview preparation, and job accommodations. To connect with CPWD’s Career Coach, please call call (303) 442-8662 x 233 to request an appointment.
The Division of Vocational Rehabilitation (DVR) has services to help you obtain or maintain a job. If you are already employed, DVR can help you access the accomodations and/or technology you may need to stay employed. DVR has an Interest & Referral Form that you can complete online, which will get you connected with a DVR counselor and on your way to job-seeking. To find your local DVR office, click here.
