Nursing Home Transitions: From Institution to Independence

CPWD assists people with disabilities to transition from nursing homes to independent living.

Institutionalized individuals have limited independence and marginal support. They are often medicalized, managed without dignity, and experience undue suffering.


CPWD’s Nursing Home Transition program employs experts in the complex process of transitioning people to independent living. From finding a home, to securing benefits, in-home care and accommodations, to providing independent living skills training, our staff assist consumers from start to finish. We also provide post-transition support and follow-up, ensuring successful and permanent transitions.


Not only does living independently provide dignity, choice, and a more fulfilling life, on average, per person, it costs $60,000 - $100,000 less in Medicaid fees and taxpayer funds. Most of all, transition services offer freedom from institutionalization and access to independent living.
