CPWD’s Services
Home Health Program
CPWD’s Home Health Program provides a wide variety of services to meet the physical needs of people with disabilities in independent living environments. Different levels of service providers manage specific services. In partnership with our care providers, consumers determine the services they need as well as the provider. A trusting relationship is an important foundation to providing services for people with disabilities in their homes.
Following is a list of the most common levels of care providers and the services they offer:
In Home Support Services (IHSS)
- Family members, roommates or friends are paid to care for their loved one
- Consumer directed – the consumer can hire and fire caregivers
- The plan of care is created with the consumer’s input
- Caregivers do not need to be licensed to do skilled – level care
- All personnel issues (training, payroll, taxes) are managed by CPWD
Personal Care Provider (PCP)
- Showers/bathing
- Medication reminder
- Oral care
- Meals
- Homemaking activities
- Exercise
Skilled Nursing
- Medication management
- General nursing assessments
- Injections
- Monitor for signs and symptoms of infection
- Evaluate diet and fluid intake
- Monitor for safety in the home
- Report changes in condition to physician
- Order medication refills and pick up from pharmacy
Certified Nursing Assistant (CNA)
- Bathing
- Toileting
- Transfers
- Bowel care and catheter care
- Medication assistance
- Feeding
- Dressing
Homemaker
- Light cleaning
- Organizing
- Cooking
- Laundry
- Grocery shopping
